Our Patient Care Navigators are integrated into your care team to provide personalized support—because you deserve care that meets your needs. If additional services or referrals are needed, we’ll work with you every step of the way.
How It Works
Care Navigators provide personalized support and help with scheduling the services you need:
- Make appointments for MFHS programs and help you to decide what programs are right for you, including referrals to our trusted Reproductive Health Network.
- Help with health insurance enrollment and/or SNAP benefits.
- Assist you with resources such as food pantries, housing, transportation or other community referrals.
- Bilingual staff can provide support in English and Spanish. Telephone translation is available for other languages.
